In Yu-Gi-Oh!, an Ultra Rare is a card printed with a gold foil name and holofoil artwork. Those two things together are the whole definition. It is a middle tier of the standard rarity order, not the top of it, and it is one of the easiest rarities in any game to confirm by eye.

Read the name, not the picture

Yu-Gi-Oh! puts most of its rarity information in the card name's foiling, which makes the name the fastest thing to check:

Common

Name in plain black text, artwork not holofoil.

Rare

Name in silver foil, artwork not holofoil.

Super Rare

Name in plain text, artwork holofoil — the opposite pairing to a Rare.

Ultra Rare

Name in gold foil and artwork holofoil.

Secret Rare

Name in holographic silver, with a diagonal sparkling foil across the artwork.

The phrase is used loosely everywhere else

⚠ Outside Yu-Gi-Oh!, "ultra rare" is generic marketing language meaning "very rare", and several games and sports products use it as an informal label with no fixed definition. Inside Yu-Gi-Oh! it is a printed, verifiable finish that sits in a known position — above Super Rare, below Secret Rare, and well below the specialty tiers. If a listing says "ultra rare" without naming a game, it is a description, not a specification.
